- Our team of licensed pharmacists dedicate 1hr of support services to each eligible Medicaid patient each month (at no cost to healthcare provider or patient)
How It Works
We complete an eligibility check to identify Medicaid patients who are eligible for the service
We match eligible patients with a pharmacist who provides 1hr of support services each month
Your most at-risk patients are empowered to lead healthier lives and your organization receives additional capacity to provide exceptional care to the most vulnerable
Services Include:
- Counseling on the importance of medication adherence
- Coordinating refills
- Coordinating free home delivery
- Providing medication education
- Developing medication treatment plans
- Monitoring and evaluating responses to medication
- Supporting patients with challenges with Medicaid coverage
- Coordinating with your staff of providers to provide exceptional care
- Assessing physical and mental health status
